New classification of migraine
The 3rd edition of the International Classification of Headache Disorders (ICHD-3) from the year 2018 made major changes in migraine classifications: renaming „basilar-artery migraine“ to „migraine with brain stem aura“; transferring chronic migraine from migraine complications into a separate clinical variation of migraine, and updating episodic syndromes related to migraine. The ICHD-3 has elaborated diagnostic criteria for the migraine with aura and its subtypes to distinguish focal reversible neurological deficits from other transient neurological disorders (transitory ischemic attacks). Criteria for the chronic migraine now includes both migraine with aura and primary headaches, in addition to the migraine without aura. Both chronic migraine and medication overuse headache may be present at the same time. In ICHD-3, there are alternative criteria for dividing episodic migraines due to their relation with the menstrual cycle and chronic migraine due to the duration of remission. The new migraine classification will enable for more reliable criteria for involving patients in scientific research and more precise diagnosis of migraine types in daily clinical practice.
